REGISTRATIONS & MEMBERSHIPS

NEW MEMBERSHIPS

1. SSA Capitation form – may be obtained from the ONYX Gala Secretary and needs to be completed and the original returned.
2. ONYX Membership & Indemnity Form – may be obtained from Angela Meehan and needs to be completed and returned.
3. ID/Birth Certificate – a copy needs to be attached for all new members.
4. ID/Passport Photo – as recent as possible for all new members.
5. Receipt of payment.

RENEWAL MEMBERSHIPS

1. SSA Capitation form – may be obtained from the ONYX Gala Secretary and needs to be completed and the original returned.
2. ONYX Membership & Indemnity Form – may be obtained from Angela Meehan and needs to be completed and returned.
3. Receipt of payment.

TRANSFERING FROM ANOTHER CLUB

1. A Clearance Certificate needs to be obtained by the swimmer and attached to all the documentation as proof that there are no outstanding funds to the previous club.
2. SSA Capitation form – may be obtained from the ONYX Gala Secretary and needs to be completed and the original returned.
3. ONYX Membership & Indemnity Form – may be obtained from Angela Meehan and needs to be completed and returned.
4. Copy of ID/Passport & Photo.
5. Receipt of payment.
